Your First Appointment
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records

Insurance and Payment Information
Dr. William J. Somers, M.D. is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
We expect payment at the time of service either in full or as designated by your insurance plan. As a convenience for you, we accept cash, check, MasterCard, Visa or Debit Cards as payment options.

Hours and Appointments
Our office hours for appointments are Monday, Wednesday and Thursday from 8:30 a.m. to 4:30 p.m.
We are open from 8:30 a.m. to 6:30 p.m. on Tuesdays and Fridays. Please tell the receptionist if you need to be seen promptly due to a problem.
If you are unable to keep an appointment, please call as soon as possible so your appointment can be rescheduled and your time may be used for another patient.
Medical Records
All of your medical records are confidential, and no information is given over the telephone without your written consent. Your written authorization is required before your records will be released to insurance companies or others. There is a minimum processing fee of $25.00 for reproduction and release of records.

Telephone
If you have non-emergency questions, or are calling to get laboratory reports, please call between 8:30 a.m. and 4:30 p.m. Monday, Wednesday and Thursday and on Tuesday and Friday from 8:30 a.m. and 6:30 p.m. when your medical records are readily available.
If you are calling for a prescription refill, please call during the office hours so we can check your records to make sure a refill is the best course of treatment. You also should have the name and telephone number of your pharmacy available before your call.
